Your first appointment at FORME is a conversation before it's anything else. We spend time understanding your history, your goals, and any concerns you have before we ever pick up an instrument.
Most new patient appointments run 60 to 90 minutes. That's intentional. We use that time for a comprehensive exam, digital X-rays if needed, and a thorough discussion of what we find and what we recommend. You'll leave with a clear picture of your dental health and a treatment plan that makes sense, not a list of procedures pushed at you.
You won't feel rushed. You won't be handed off to an assistant mid-conversation. You'll be seen by Dr. Voong or Dr. Fung directly, start to finish.
How It Works
From the moment you book to the moment you leave, here's exactly what happens.
Schedule through our online booking system. You'll receive a confirmation and a short intake form to complete before your visit, which takes about 5 minutes.
Take the elevator to Suite 1503. We'll greet you by name. The space is calm and unhurried, deliberately designed to feel nothing like a traditional dental office.
Dr. Voong or Dr. Fung will spend time getting to know you: your history, your goals, any anxiety or concerns. We listen before we recommend.
A thorough evaluation of your teeth, gums, bite, and overall oral health. Digital X-rays if needed. We explain everything we're seeing in plain language as we go.
We present a clear, prioritized plan with costs outlined upfront. Nothing is scheduled without your full understanding and agreement. No pressure, ever.
Take the plan home, think it over, ask questions. We're here when you're ready. Many patients schedule their next visit before they leave, but only when they feel ready.
Getting Here
FORME Dental is located at 575 Madison Avenue, Suite 1503 in Midtown Manhattan, in the heart of the 10022 zip code, steps from Park Avenue and 56th Street.
By subway: The 4, 5, 6 trains stop at 59th Street/Lexington Avenue (4 blocks away). The E, M trains stop at Lexington/53rd Street (3 blocks away). The N, R, W trains stop at 5th Avenue/59th Street.
By bus: The M1, M2, M3, M4 buses run along Madison Avenue and stop directly in front of the building.
Parking: Several parking garages are available within one block on 55th and 56th Streets. Street parking is limited in Midtown but available on side streets.
Hours: Wednesday and Thursday 9am to 6pm, Saturday 9am to 3pm by appointment.

Dental Anxiety
You're not alone, and you won't be judged here. Dental anxiety, whether mild nervousness or genuine avoidance, is something we see regularly, and something we take seriously.
We move at your pace. We explain before we act. We stop if you need a moment. If it's been years since your last visit, we approach your care with patience, not urgency. Our first goal is simply to make you comfortable enough to come back.
